To access immunization information visit the Arizona Department of Health Services (ADHS) webpage. Please note this may not be a complete immunization record due to limitations with reporting to ADHS.
FollowMyHealth Patient Portal
You can manage your child’s personal health information with 24/7 secure, online access from any computer, smartphone or tablet.
Through a partnership with FollowMyHealth™, we are offering parents access to their children's health information.
NOTE: Not all information is available in the patient portal. To request your children's full medical records, please follow the instructions, below.
Requesting Patient Medical Records
To receive your child's medical records, you must complete the authorization form:

Processing medical record requests may take seven to ten business days from the date of receipt. Records will be released and delivered in the method indicated on the authorization form (fax, email, via patient portal). Occasionally delays occur due to high volumes. Some requests may take up to 30 days to be processed. Radiology images will be released through Powershare or burned onto a CD.
The completed form along with a valid copy of your photo ID can be faxed, emailed as an attachment, mailed or dropped off at the front desk of any of our locations. If you are not the biological parent, please submit a copy of legal guardianship paperwork along with your request.

Our third-party vendor called Verisma processes all legal requests. To check the status of these requests, please contact Verisma at 866-390-7404.

To avoid faxing to Health Information Management (HIM) or delays in receiving records, HealthPoint is an electronic method for referring physicians to view the records, schedule appointments, surgeries, and/or diagnostic testing of a patient.
Health Information Exchange (HIE) — Health Current
Health Current is for providers to access patient health information electronically in a secure and timely manner.
